Donor registration

We are delighted that you are interested in our work at the Multiple Sclerosis and Parkinson's Tissue Bank. You may be considering becoming a tissue donor - information brochures are available for both Multiple Sclerosis and Parkinson's via the download buttons to the right, which aim to answer any questions you may have about the process.

If you want to go ahead and request the documents needed to register as a donor, complete the short form below.

By completing the request form above, you are consenting for us to send you registration documents and information on becoming a donor. When you submit the request form above, one of our representatives will email you the necessary registration documents to complete (postal copies are also available on request).

Please note that data inserted into this form is collected via Qualtrics. Your information will be held on a database and will be used to contact you with further information from Imperial College London. See the Imperial College London Data Protection Policy.
